Download and Print New Patient Registration
After you schedule your appointment with your Pediatric Dentist of choice, in Doral or Coral Gables, we encourage you to download and print our new patient forms, so that you can fill them out at your convenience. You can either fax or mail to us or bring them with you to your first appointment.

Get Directions
We have two convenient locations with great teams of Pediatric Dentists in South Florida, one in Doral and another one in Coral Gables. Pick the closest to you. Get directions on our locations page, or you may call our office to get more detailed instructions.
